There are a number of choices to make in the Forms Setting.
Response Settings
There are two response settings available. Below is an explanation of each:
- Accepting Responses
- If boxed is unchecked, the Form is unable to accept any responses.
- Limit number of form entries to 1 per user
- If boxed is checked, each user can only submit the form 1 time.
- If boxed is unchecked, users can submit responses to the form as many times as needed.
Permissions
Permissions control the following:
- Who the forms can be delivered to & who can fill out the form.
- Who can edit and make changes to the form.
- Who will receive the responses to the form when filled out.
Below is a summary for each Permissions Section in the Settings Tab:
- This Form Applies to: Select who you want the form to be delivered to and filled out by. You may select more than one group to deliver a form to.
- Staff
- Only Staff Members will have access to fill this form out
- Student
- Only Students will have access to fill this form out
- Family Members
- Only Family Members will have access to fill this form out.
- Staff
- Form editors: Who can edit and make changes to the form you have created. You can leave blank if you do not want others to edit the form you've created.
- Type in each staff member email address here. Separate multiple staff members with a comma.
- Share responses with: Who will receive responses to the form once it has been filled out.
- Type in each staff member email address here. Separate multiple staff members with a comma.
Messages
Upon completing/submitting a form- your staff, students, or family members can receive a message. This message can be customized by the form creator.
- If left blank, a generic message will be created ("Your response has been recorded")
- Receive Email Notifications for new responses
- If checked, the form creator will receive email notifications that the form has been filled out by a user.
- If unchecked, the form creator will not receive email notifications.
Visibility
Make this a recommended form
A form will be available for the various user groups to use (ie all, staff, family members, students).
Make this form a district template
Templates can be copied by users. The form is a template to other forms. For example, a district may want to create a template form called "family member / student communication form". By making this form a template, teachers can copy the same form framework. But, given the form is technically a DIFFERENT form, results will not be aggregated.
Make this form a plan
The form will become part of the plans app. For example, a district may want to create their own version of a 504 plan. They would:
- Create a 504 plan in forms
- Select "Make this form a plan"
- Access the form (to complete) in the Plans App
Of Note
For most teachers, they will not edit form visibility.
